It has been developed by the Church of England's Life Events team and updated to reflect changes to marriage registration introduced in May 2021.
It is ideal for use alongside other Church of England Life Events stationery items such as the Weddings Couple's Information Pack.
Each pack of 30 forms is supplied in a resealable bag to ensure they can be stored and kept in good condition.
